Specifically, the E.D.S. Plan provides the following benefits for each of your family members at 100% when visiting in-network providers!

• Filling (two surfaces) - once a year

The Dental Plan provides access to 75,000 credentialed dentists nationwide. You can locate a provider via the website or by calling Membership Service toll free at 877-OK-COGIC. By seeing a network provider, the insured services listed above are covered 100%. For treatment at a non-participating provider, you'll receive benefits for these covered services based on a Maximum Allowable Charge (MAC). Members are responsible for all charges above the MAC.
